So First, let’s start with the basics: What is PIQ in SSB? After being screened in during the SSB Interview, the first thing to do is fill in the PIQ Form. PIQ Full Form in SSB is – Personal Information Questionnaire (PIQ) Form. It has been said that a well-filled PIQ form helps the assessors build a personality of the individual which is about 40% easier to achieve and rely upon.
You would be required to do PIQ form filling in two sets – one would be sent to the Psychologist and the other would go to the Interviewing Officer (IO). The GTO (Group Testing Officer) does not have to have your PIQ form as they only assess your dynamic behaviour to complete a test.
The PIQ in SSB forms an imperative steppingstone in developing your assumed persona which will, in turn, be displayed during your psychological tests and prior to your interview.
You will notice at the top right of the form, your OIR grading (i.e. II, III etc.) in red ink and in a circle.

- Maximum Residence along with district and State with approximate population.

- Permanent Residence along with district and State with approximate population.

- Mother Tongue
- DoB
- Married/ Unmarried/Widower (for SCO entry, PCsl, Women entry)
- Is your Parents Alive fill specified way
- If not alive, your age at the time of Mother’s / Father’s death fill specified way
- Parent’s / Guardian’s and Siblings’ Occupation / income (as applicable)
- Along with monthly income, and if it is in business then the annual income is divided by 12.
- For mother working or non-working as homemaker and for salary mark NA
- Guardian is to be filled only in the case of parents who are not alive.
- Elder Brother / Sister
- Younger Brother / Sister
- Educational Record (commencing from Matriculation / Equivalent Examination) :-school, board, division, percentage, medium, boarding or day scholar.
- In the achievements section : write what you achieved by your effort which gives you recognition.

- Age in years and months with height, and weight in kg.
- Present Occupation and personal monthly income, if any in 9th column
- Next is NCC Training with duration with wing and division, and certificate.
- Thereafter participation in games and sports: games are played in teams and sports are played in singles and doubles. A game improves your social adjustment, and sports improves your fighting spirit.
- Along with the games and sports you enjoyed or played you must know about the details of that. About the dimension of the field in which the game or sport is being played. Along with the famous terminologies. What is the national status of that game or sport? What are the tournaments belonging to that? Prominent players. Duration of participation in years, Representation level- School / College / University / Other. Outstanding achievement, if any.
- Hobbies and Interest
- Hobbies to fill in the PIQ Form, since hobbies are considered to be a regular activity that is done for enjoyment, typically during one’s leisure time. This is an ongoing process. Hobbies help you to keep away from negative thoughts. A good hobby makes you mentally healthy.
- Participation in extra-curricular, activities
- In SSB means to check what are the opportunities the youngster had come across and would have come across in his / her life and how many were availed by him or her or how many were missed by him.
- Duration in years, with achievements.
- Position of responsibility/ office held in N.C.C./ Scouting
- Sports Team/Extra-curricular group and other fields
- What are the responsibilities you have taken?
- In Nature of commission in PIQ Form – Permanent Commission / SSC
- Choice of Service – Army/AF/Navy any one
- Number of chances availed for commission in all three Services with detail.
- Details of all previous interviews, if any (Army / Navy / Air Force Selection Boards)
- With type of entry, SSB No. & Place, date of reporting, Chest No. / Batch No.
